FDA sorts every recall by how serious the risk is.
| Class | What FDA means by it | 4health |
|---|---|---|
| Class I | A reasonable chance the product causes serious illness or death. | 1 |
| Class II | May cause temporary or treatable illness, or the chance of serious harm is remote. | 1 |
| Class III | Unlikely to cause illness, but breaks FDA rules (labeling, for example). | 0 |
This lists the 4health recalls in FDA's Enforcement Report since 2012. Older recalls aren't included, and a very recent one can take weeks to appear. FDA records name the firm, not the brand, so every match here was checked by hand against the product description. Few or no recalls in these records doesn't mean a brand hasn't had any safety issues, only that FDA hasn't recorded a recall for it in this period.
